Paraclinoid internal carotid aneurysms arise from the internal carotid artery between the distal dural ring and the posterior communicating artery. Surgical treatment of these aneurysms is technically challenging due to the surrounding neurovascular structures. There are several methods to treat these lesions: direct clipping with retrograde suction decompression, endovascular coil embolization, coil embolization with stents, and flow-diverter stents. Here, we briefly review the surgical and endovascular treatment options.
